NYS Jail Medical-Provider Fitness Directives: Statewide Research Dataset
Transparent Law Enforcement | October 18, 2026
Transparent Law Enforcement (TLE) reviewed 96 New York State Commission of Correction reports concerning county jails outside New York City to identify directives instructing county governing bodies to examine the fitness of their jail medical providers.
The review also examined publicly available county legislative records and news reporting to determine what could be independently established about county responses.
Research findings
Of the 96 Commission reports reviewed:
- 36 reports directed a county governing body to review findings and conduct an inquiry into the fitness of a designated medical provider.
- 20 counties received at least one such directive.
- Two additional reports used different wording and are classified separately.
- 11 of 22 reports issued in 2025 contained a standard fitness directive.
The directives involved both contracted medical providers and county-operated medical services. A fitness directive does not necessarily mean the Commission concluded that the death was preventable.
TLE could not independently locate, for the 19 counties other than Monroe, a publicly documented, completed governing-body fitness inquiry establishing its scope, evidence and conclusions.
Some counties reported actions or reviews, including medical-provider changes. Those actions are recorded in the dataset where identified. The absence of accessible documentation does not establish that an inquiry never occurred.

Research coverage: Commission reports published through October 2026; county public-source review through October 18, 2026.
Methodology and limitations
TLE reviewed every report concerning a county jail outside New York City listed on the Commission's Incarcerated Mortality Reports page during the research period. The 96 reports concern 95 deaths and one investigation involving a person who survived.
All 38 reports classified as standard directives or differently worded variants, along with five sampled reports without such directives, received a second independent classification review.
County follow-through research used publicly accessible legislative minutes, agendas, resolutions, procurement materials and local reporting. Not all county records were available online. The searches were not exhaustive, and TLE did not submit Freedom of Information Law requests for this research.
The dataset covers reports publicly posted by the Commission, not every death in county custody or every directive the Commission has issued. Report summaries are abbreviated and should be checked against the linked originals before quotation.
Research Correspondence
October 18, 2026 — Submission to Monroe County Legislature Special Committee
Transparent Law Enforcement submitted a letter to the Legislature's special committee examining the fitness of the Monroe County Jail's medical provider. The submission requests that the committee's eventual public report document the scope of its inquiry, the evidence considered, the basis for its conclusions, and any unresolved questions.
